Before patients can legally purchase or possess medical marijuana in Ohio; Patients must be registered with the Ohio State Board of Pharmacy. In order to get registered:

  1. Patients must receive a diagnosis from a recommending Physician which confirms one of the 21 qualifying conditions.

  2. Once diagnosed, patients must consent to treatment with medical marijuana.

  3. If under 18 years old, your parent or legal guardian must also register with the Ohio State Board of Pharmacy; and consent to treatment with medical marijuana. 

  4. Pay required fees to the Ohio State Board of Pharmacy. 


Patient's and/or Caregiver's with an active Ohio Medical Marijuana Card can enter the dispensary during business hours.

During your first visit, you’ll need to bring the following documents:

  • Your active patient ID card (printed or electronic)

  • Valid government-issued photo ID (state issued driver's license or ID, passport)

  • Alzheimer’s Disease

  • Amyotrophic Lateral Sclerosis

  • Cancer

  • Chronic Pain

  • Chronic Traumatic Encephalopathy

  • Crohn’s Disease

  • Epilepsy or another seizure disorder

  • Fibromyalgia

  • Glaucoma

  • Hepatitis C

  • HIV+

  • Inflammatory Bowel Disease

  • Multiple Sclerosis

  • Parkinson’s Disease

  • PTSD

  • Sickle Cell Anemia

  • Spinal Cord Disease/Injury

  • Tourette’s Syndrome

  • Traumatic Brain Injury

  • Ulcerative Colitis


If your family member or friend is unable to physically go to a medical marijuana dispensary, you may be  apply to be a licensed caregiver (and serve as a caregiver for up to two patients). Caregivers are essential to medical marijuana treatment programs for those suffering from terminal illnesses, and are legally able to assist patients in administering their medication. To become a caregiver, you must be over 21 years of age. Visit your friend or family member’s recommending physician with valid photo ID and the required $25 fee to have them submit your caregiver registration.

In order to keep your registration active, you must renew your registration annually before the expiration date. Your referring physician must submit your renewal application up to one month before the expiration date.

The Board of Pharmacy will send a notice to the address you provided in your application 45 days before your card expires.  If your expiration date is less than 30 days away, or if it has already passed, contact your physician for an updated recommendation and reapply for a new ID card. Active patient registration expires one year after the day it’s issued, on the last day of the month it was issued.
